PORT ANGELES — North Olympic Healthcare Network has received two American Heart Association outpatient program achievement awards.
The awards, North Olympic Healthcare Network (NOHN), said in a press release is “in recognition of its commitment to ensuring patients have access to the latest evidence-based practices that reduce the risk of heart disease and stroke by improving Type 2 diabetes and blood pressure management.
Nearly half of all adults in the U.S. have some form of cardiovascular disease, including heart attack, stroke, or heart failure, NOHN said, citing Benjamin et al. “Heart Disease and Stroke Statistics—2019 Update: A Report from the American Heart Association. Circulation.”
Type 2 diabetes and uncontrolled high blood pressure are leading risk factors for cardiovascular disease, “but with timely diagnosis, research-based treatment and education, these conditions can be managed,” it said.
“We are proud to be recognized by the American Heart Association for our focus on high blood pressure and Type 2 diabetes management and care,” said CEO Dr. Michael Maxwell.
“By participating in these programs, we are able to put the science of the latest clinical guidelines to work improving the lives of our patients.
“These national recognitions reflect the hard work and dedication of our medical providers, clinical support staff, and quality improvement team,” Maxwell said.
This year, NOHN received these achievement awards:
• Target: BP™ Gold+ recognition jointly presented by the American Heart Association and the American Medical Association for demonstrating a commitment to measurement accuracy and helping to control high blood pressure in at least 70 percent of hypertensive patients.
• Target: Type 2 Diabetes™ Gold recognition jointly presented by the American Heart Association and American Diabetes Association for meeting specified thresholds for two clinical measures while improving quality of care through awareness, detection, and management of Type 2 diabetes by educating and empowering patients with evidence-based information and tools.
The 2022 Target: BP™ and Target: Type 2 Diabetes™ awards are based on clinical outcomes for the Jan. 1 to Dec. 31, 2021, reporting period.
NOHN is a federally qualified health center (FQHC). It said in the press release that it serves more than 16,000 patients on the North Olympic Peninsula.
It provides primary care, obstetrics, pediatrics, and patient navigation/assistance at its Downtown Health Clinic, 240 W. Front St., in Port Angeles, as well as behavioral health services and dental care at its Expanded Services Building, 933 E. First St., in Port Angeles.
