PORT ANGELES — The North Olympic Health Network recently received a $78,000 grant from PacificSource Health Plans’ Community Health Excellence grant program.
The network will use the funding to expand its care coordination team from two to four full-time employees.
Care coordinators assist with managing transitions of care, addressing care gaps, identifying social drivers of health and integrating additional services such as patient navigation, behavioral health, oral health, pharmacy and optometry.
The new care coordinators will focus on providing transitional care management for patients recently discharged from the emergency room.
“Care coordination is key to improving health outcomes for our under-served and elderly patients, particularly those with chronic conditions,” said Dr. Michael Maxwell, CEO of the North Olympic Health Network.
“We’re grateful for the funding we’ve received from PacificSource to better serve these patients and pleased to welcome Tiana Taylor, MA, to the NOHN family as our first PacificSource grant-supported hire.”
The annual grant program has provided more that $11 million in grant awards to advance healthcare delivery innovations in Washington, Idaho, Montana and Oregon.
